Rise SC
Founder of Rise SC, Jasmine Guest-Sanders is a South Carolina native and health equity advocate who launched this initiative after witnessing how hard it can be for families to find trusted care and resources for chronic disease. With years of experience in grant management and community health, she created Rise SC to connect patients, families, and clinicians across the state through access, education, and support.

What is Rise SC?
Rise SC is a pilot Sickle Cell Resource Hub led by J Creative Consulting. Our goal is simple: to make it easier for families, caregivers, and providers in South Carolina to connect with the resources, programs, and support systems that already exist — without the confusion of searching across scattered sites and organizations.
Why Rise SC Matters
More than 4,000 South Carolinians live with sickle cell disease. While excellent programs are available through hospitals, nonprofits, and the state, families often face a maze of disconnected information. Rise SC was created to close those gaps by linking resources together in one clear, accessible hub. With trusted guidance, local connections, and plain-language tools, Rise SC serves as a front door to care, support, and community.
